About the Role

The Case Worker role provides person-centred, strengths-based support to young people aged 10–24, with a focus on homelessness and early intervention. Working across multiple LGAs, the role delivers assertive outreach and evidence-informed case management, supporting clients to achieve sustainable housing, build capacity, and access appropriate services. The position requires strong relationship-building, sound judgement, and the ability to respond flexibly to the complex needs of vulnerable young people in community settings.

Responsibilities

  • Work with and respond to referrals of clients across South West Sydney providing ongoing case management and support
  • Create individualized support plans including referrals to community services organisations
  • Promotion of the service in the local community
  • Monitor our clients progress towards achieving their goals

Requirements

  • Community Services Degree; or Associate Diploma or certificate with relevant experience
  • Strong communication, empathy and advocacy skills
  • Have a sound understanding of Mission Australia’s values and code of conduct
  • Well-developed written and oral communication skills, including highly developed administrative skills
  • Current drivers licence
  • Undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people
