About the Role

We are seeking a dedicated and empathetic Social Worker to deliver a wide range of psychosocial services, ensuring holistic care and emotional support for those in need. Responsibilities

  • Conduct initial psychosocial evaluations of patients and their families to assess needs and develop appropriate care plans
  • Provide ongoing psychosocial counseling to support emotional well-being and coping strategies
  • Deliver direct casework services, including patient and family education, advocacy, and resource coordination
  • Assist with discharge planning to ensure smooth transitions and continued support post-hospitalization
  • Offer bereavement services to families experiencing loss, providing comfort and guidance
  • Engage in community education, outreach, and referral activities to promote awareness and access to hospice services
  • Collaborate with the interdisciplinary team to develop and implement individualized plans of care

Requirements

  • Masters’ degree in Social Work from an accredited educational institution
  • LMSW or LCSW preferred
  • One year experience in individual, family and group counseling, preferably in a healthcare or hospice setting
  • MSW candidates will have 6 months to obtain LMSW
  • Possess appropriate certificate / license to practice as a Licensed healthcare professional in the State of New York
  • Certification in Hospice & Palliative Care is preferred if offered for discipline
  • Valid NYS driver’s license required

Experience Level

  • One year experience in individual, family and group counseling, preferably in a healthcare or hospice setting

Education Level

  • Masters’ degree in Social Work from an accredited educational institution
