About the Role
CASES is seeking a Clinical Case Coordinator to join the Pretrial Services Case Management Team. This role involves conducting Supervision Introduction/Orientation meetings, assisting participants with compliance to supervision and court appearances, and developing collaborative service plans. The goal is to increase engagement in community treatment services and support participants in achieving their goals for treatment, recovery, rehabilitation, and crime-free community integration.
Responsibilities
- Provide supervision and community-based services to a caseload of pretrial participants.
- Conduct Supervision Introduction/Orientation meetings with new participants within 24-48 hours of release to review supervision conditions, reporting schedule, and communication methods.
- Collaborate with the court and pretrial staff to ensure participants complete mandated supervision appointments and court appearances.
- Review legal and court screening documentation to analyze offending behavior patterns and identify changes needed to reduce pretrial arrests.
- Conduct ongoing service need assessments to examine psychosocial needs, including mental and substance use disorders, and participant motivation for treatment.
- Link participants to treatment based on eligibility, preferences, and needs, and provide escort to community services.
- Ensure timely flow of community outreach and case management activities to meet program goals.
- Utilize problem-solving techniques and motivational interviewing to engage individuals in processes of change.
- Provide services in a gender-responsive, trauma-informed manner adhering to risk-need-responsivity principles.
- Maintain collaborative relationships with community providers (e.g., housing, health homes, care management, addiction/mental health, peer-run, education, vocational, legal, medical, family).
- Follow program community-based monitoring protocols for high-risk and high-need participants and participate in case reviews.
- Enter assessment, supervision reporting, and progress notes into the program database, adhering to confidentiality regulations.
- Meet monthly case management contact and outreach goals.
- Provide culturally competent services in accordance with CASES policies.
- Perform other duties as required by supervisory staff.
- Demonstrate flexibility with schedule based on departmental needs.
Requirements
- Master’s degree in Social Work, Mental Health Counseling or comparable professional degree.
- Licensure preferred.
- Minimum of three years’ experience working in human services with individuals with substance use disorders, co-occurring mental health and substance use disorders, and involvement in the criminal justice system.
- Extensive knowledge of community treatment, support services, and resources.
- Knowledge of the criminal justice system, court processes, and alternative to detention/incarceration services.
- Experience using databases such as Salesforce.
