About the Role

Youth Assertive Community Treatment (ACT) services aim to improve functional impairments and severe symptomatology in youth due to mental illness or serious emotional disturbance. Interventions focus on enhancing family functioning for stability and reintegration of youth returning from residential treatment or inpatient hospitalization. The Youth ACT Team is a multi-disciplinary team providing family-driven, youth-guided, and developmentally appropriate services to address youth needs across multiple domains.

Responsibilities

• Use personal experience within youth peer advocacy interventions with families.
• Provide advocacy with providers across the child-serving system to raise awareness, reduce stigma, engage families, and coordinate services.
• Educate youth about self-help techniques and group processes.
• Provide psychoeducation to youth in treatment.
• Provide individual skill development to youth related to behavioral health needs.
• Teach effective coping strategies based on personal experience and assist in developing community support systems and networks.
• Support youth in developing skills to effectively manage behaviors and navigate multiple systems.
• Identify formal services and informal resources for youth experiencing social-emotional, behavioral, or mental health challenges.
• Assist youth with identifying challenges, strengths, and areas for improvement/goals.
• Monitor and document youth progress in accordance with agency and regulatory policies.
• Collaborate with care providers and community supports to help youth track progress towards goals.
• Participate in multi-disciplinary team meetings, staff meetings, training, and supervision.
• Maintain all documentation according to established standards and time frames.
• Maintain professional behaviors and ethical standards.
• Document demographic data, family goals, and services provided using an electronic database.
• Participate in quality improvement activities.
• Perform services in the family’s home, youth’s home, community, or office space.
• Perform other tasks and duties as assigned.

Requirements

• High School Diploma or Equivalency.
• Specialty credentialing in advocacy (Provisional) can be obtained during the probationary period.
• Certification must be obtained within one year.
• Lived experience of mental health challenges.
• Background in advocating mental health and/or in the educational system.
• Working knowledge of Microsoft Office.
• Ability to learn electronic health records and other software.
• Ability to read paper and electronic documents and perform data entry.
• Manual dexterity and hand-eye coordination to travel independently using public transportation.
• Ability to travel throughout New York City carrying equipment such as a laptop and cell phone weighing up to approximately 10 pounds.
• Routinely required to sit (30% of the time), stand (35% of the time), and travel to and from appointments using varied public and private transportation options (35% of the time).
