About the Role

The Case Associate functions as a Case Planner, responsible for supplying appropriate interventions to ensure client care. This role involves working within the client's community to develop action plans, identify and utilize existing resources, and establish new ones for client care.

Responsibilities

  • Perform routine community contacts on behalf of social workers, such as school and medical reports.
  • Locate and utilize appropriate community resources for clients.
  • Evaluate actual living conditions of clients through home visits.
  • Interpret agency functions to clients and demonstrate concrete help.
  • Provide structure to clients, evaluate and assess the usage of independent living skills, and offer guidance.
  • Run groups under the supervision of social workers.
  • Evaluate clients for individualized treatment programs, including child safety, in concert with the Sr. Case Planner.
  • Understand the needs of the individual client, family, and their support network.
  • Identify appropriate treatment or service providers and facilitate client referrals to internal, external, and community linkage programs.
  • Perform individual and group counseling, varying in duration based on client needs.
  • Conduct home visits to assess the needs of the family and child safety.
  • Identify and help clients obtain needed services, such as maintaining medical, financial, and social resources.
  • Provide case management to clients.
  • Identify and provide access to specific appropriate concrete services when necessary or requested and in the client's best interest.
  • Perform community outreach and establish links with helpful groups, individuals, or organizations.
  • Provide all necessary notes and progress notes.
  • Perform risk assessments.

Requirements

  • Ability to engage families and teens of all backgrounds and cultures in their home and community.
  • Knowledge of Microsoft Office required.
  • Limited applications of manual dexterity and eye-hand coordination.

Experience Level
