About the Role

Outreach is seeking an experienced Registered Nurse (RN) to work in our outpatient facility. The RN assesses health-related needs, monitors client wellbeing, and coordinates health services. This role includes on-call nursing and serving as the healthcare coordinator.

Responsibilities

  • Conduct assessments of substance abusers and family members to determine treatment appropriateness and need for medical services.
  • Follow up on health-related referrals.
  • Maintain a tracking system for patient health issues, medical appointments, and prescribed medications.
  • Provide patient and staff education on health issues.
  • Provide crisis intervention on an emergency basis.
  • Complete all documentation and reports in a timely manner, informing leadership of any issues.
  • Participate in staff meetings, case conferences, and unit meetings.
  • Maintain communication and teamwork with clinical staff.
  • Conduct toxicology testing as needed.
  • Identify and address the health needs of the patient population.
  • Deliver quality care in line with the agency's Medicaid Compliance Plan.
  • Report any violations of the Medicaid compliance plan or personnel policies.
  • Serve as a designee for the Medical Director to authorize urine and oral fluid testing.
  • Interact with staff, clients, students, and visitors courteously and professionally.
  • Produce accurate, neat, and thorough work.
  • Prioritize and plan work time efficiently, managing competing demands.
  • Comply with agency policies and procedures.
  • Support a positive work atmosphere and get along with clients, students, coworkers, and supervisors.
  • Communicate clearly and concisely, both orally and in writing.
  • Demonstrate flexibility and adaptability in role and function.
  • Maintain appropriate boundaries, ethics, and professionalism, seeking supervision when needed.

Requirements

  • NYS Registered Nurse (RN)
  • Previous experience working as a Registered Nurse in a substance use disorder treatment facility setting preferred.
  • Previous experience working as a Registered Nurse in a related treatment, therapeutic community, community residence, and/or mental health setting working directly with special populations required.
