About the Role

Provide ongoing support to individuals, promoting their active participation in daily life activities such as self-care, health, safety, household chores, socialization, and overall well-being to ensure a high quality of life.

Responsibilities

  • Schedule, coordinate, and attend medical appointments, ensuring follow-through.
  • Communicate medical information to the Interdisciplinary team and family as directed by the Nurse.
  • Assist with medication administration following agency policy and procedures.
  • Complete weekly medication inventory and order medications.
  • Maintain organization and cleanliness of the medication room.
  • Review medical books and MARs with the Nurse.
  • Communicate daily with Nursing and Management regarding residents' medical needs.
  • Provide transportation for medical appointments, shopping, recreational outings, and other needs.
  • Follow all treatment plans and utilize adaptive and safety equipment to promote independence and ensure health and safety.
  • Implement and document treatment plans and agency materials to assist individuals in achieving independence and ensuring health, safety, and well-being.
  • Adhere to overtime policies to ensure quality support.
  • Participate in inter-disciplinary team meetings to discuss progress, goal achievements, and share information.
  • Ensure a safe and clean home environment by assisting with and participating in household chores.
  • Mentor new Direct Support Staff by demonstrating routines and reviewing training manuals.
  • Perform other duties as requested by supervisor.
  • Demonstrate effective communication and interaction with diverse populations.
  • Manage personal behaviors to create an inclusive, equitable, and welcoming environment.
  • Deliver culturally responsive services.

Requirements

  • High school diploma or equivalent required.
  • Qualified NYS driver's license.
  • Ability to communicate and understand written and oral directives/information.
  • Ability to lift and transfer individuals, with or without accommodations.
  • Subject to successful completion of fingerprint/criminal background check due to regular and substantial unsupervised physical contact with individuals receiving services.
