About the Role

Oak Street Health is seeking a Clinical Informatics Specialist (Medical Scribe) to support primary care providers by managing clinical documentation, allowing providers to focus on patient care. This role involves assisting providers throughout the patient journey, from morning huddles to in-exam room documentation and post-visit follow-up. Scribes become experts in value-based care models and chronic condition documentation, using this knowledge to help close care gaps. Extensive on-the-job training is provided.

Responsibilities

  • Document patient encounters in real-time, including history, assessment, plan, and physical exam.
  • Assign appropriate CPT and ICD-10 codes.
  • Prepare After Visit Summaries.
  • Consult with providers to ensure accurate and specific documentation.
  • Request and review medical records.
  • Utilize population health tools to support clinical documentation improvement.
  • Prepare for and support Daily Huddles and Clinical Documentation Reviews.
  • Consult with providers on clinical documentation opportunities.
  • Place orders and referrals.
  • Address tasks.
  • Support the care team with additional clinical documentation responsibilities.
  • Perform other duties as assigned.

Requirements

  • Knowledge of medical terminology and common medications.
  • Prior clinical experience, including shadowing and/or volunteering is strongly preferred.
  • Prior scribe or transcription experience is preferred but not required.
  • Advanced listening and communication skills.
  • Strong computer literacy and ability to learn new technical workflows.
  • Fluency in Spanish, Polish, Russian, or other languages spoken by communities served is required where indicated.
  • Ability to adapt to new workflows and quickly learn new concepts and skills.
  • Ability to type 70+ words per minute is strongly preferred.
  • Ability and willingness to take direction and be a team member.
  • Excellent job attendance and ability to work in-person in clinics.
  • Ability to commit to at least 1 year in the role (2+ is ideal).
  • Ability to work approximately 40-45 hours per week during clinic hours.
  • Compliance with hospital and Oak Street Health policies, including HIPAA.
  • US work authorization.
