About the Role
We are seeking an experienced and compassionate Nurse Practitioner (NP) to join our growing team in a hybrid role providing both Primary Care and Urgent Care services. This position offers the opportunity to deliver comprehensive, patient-centered care across the continuum, managing chronic conditions while also evaluating and treating acute illnesses and injuries. The ideal candidate is comfortable working in a fast-paced environment, demonstrates excellent clinical judgment, and is committed to providing high-quality, accessible healthcare to a diverse patient population.
Responsibilities
- Provide comprehensive primary care services for pediatric (if applicable), adult, and geriatric patients.
- Perform annual wellness visits, preventive screenings, and routine physical examinations.
- Diagnose, treat, and manage acute and chronic medical conditions.
- Develop individualized treatment plans and provide patient education.
- Order, interpret, and communicate laboratory and diagnostic test results.
- Prescribe medications and monitor treatment effectiveness.
- Coordinate specialty referrals and follow-up care.
- Evaluate and treat patients presenting with acute illnesses and minor injuries.
- Diagnose and manage conditions including respiratory infections, influenza, COVID-19, urinary tract infections, skin conditions, gastrointestinal complaints, musculoskeletal injuries, and other urgent medical concerns.
- Perform minor office procedures including wound care, laceration repair, incision and drainage, splinting, foreign body removal, and abscess management (within scope of practice).
- Interpret basic diagnostic testing including EKGs, point-of-care testing, and imaging results as appropriate.
- Determine appropriate disposition, including emergency department referrals when indicated.
- Maintain accurate and timely documentation in the electronic medical record (eClinicalWorks preferred).
- Collaborate with physicians, specialists, nursing staff, and care management teams to ensure coordinated patient care.
- Participate in quality improvement initiatives and population health programs.
- Ensure compliance with organizational policies, evidence-based clinical guidelines, and regulatory requirements.
- Educate patients and families on disease prevention, wellness, medications, and treatment plans.
Requirements
- Master's or Doctorate degree from an accredited Nurse Practitioner program.
- Current New York State Nurse Practitioner license.
- National Board Certification (AANP or ANCC).
- Current DEA registration and New York State Controlled Substance Registration.
- Minimum of 2 years of Nurse Practitioner experience in Primary Care, Family Medicine, Internal Medicine, Urgent Care, or Emergency Medicine.
- Current BLS certification.
- Strong clinical assessment, diagnostic, and procedural skills.
- Excellent communication and patient education abilities.
- Experience with eClinicalWorks (eCW).
- Bilingual (Spanish or another language) preferred.
- Experience treating patients across all age groups.
- ACLS certification.
