About the Role

The Medical Coordinator is responsible for coordinating medical and psychiatric services for adults with developmental disabilities, supporting them in achieving their personal outcomes.

Responsibilities

  • Ensures that the rights, health, safety, and personal outcomes of residents are supported.
  • Coordinates and schedules all medical and psychiatric services, including routine, follow-up, and emergency care.
  • Confirms appointment locations, obtains vehicles, and plans travel for appointments.
  • Ensures all necessary information is available before appointments.
  • Obtains completed consultation forms and prescriptions from physicians.
  • Actively listens to medical professionals and asks clarifying questions.
  • Reviews the medical appointment calendar daily.
  • Completes documentation of treatment and communicates it to team members.
  • Maintains medication inventory and orders medications as needed.
  • Sets up and manages paper or electronic filing systems.
  • Operates electronic systems and coordinates information flow.
  • Ensures all required documentation is completed accurately and on time.
  • Communicates respectfully with all individuals.
  • Complies with confidentiality policies.
  • Meets safety training requirements and adheres to safety policies.
  • Participates in required events, meetings, reviews, and committees.
  • Meets all job requirements and stays in compliance.
  • Adheres to FREE’s standard of ethical behavior.

Requirements

  • High school diploma or equivalent required.
  • Minimum of one (1) year of experience providing services to individuals with disabilities.
  • Valid AMAP certification required or must be obtained within the first 120 days.
  • Proficient computer skills.
  • Proficient writing skills with a grasp of basic grammar and spelling.
  • Demonstrated ability to work in a team environment with flexibility, keeping others informed, and anticipating needs.
  • Must be efficient, well-organized, and possess excellent communication skills.
  • Must be at least 18 years of age or older.
  • Valid driver’s license and satisfy criteria to be an agency driver required.
  • Employee is required to sit, walk, and stand for prolonged periods (at least 8 hours).
  • Employee is required to effectively communicate both verbally and in writing.
  • Employee is required to operate standard office equipment.
  • Employee is required to reach with hands and arms.
  • Employee is required to bend, stoop, kneel, crouch, or stand on a stool as needed.
  • Employee is required to be able to lift 50 pounds at a time.
