About the Role

This position is responsible for coordinating services for recovery, employment, education, support groups, and housing within agency Reintegration programs, under the intensive clinical supervision of an OASAS approved Qualified Health Professional (QHP).

Responsibilities

  • Documents individual progress in accordance with agency policy and procedure.
  • Prepares discharge summaries in accordance with agency policy and procedure.
  • Provides case management, including enhancement of adult daily living skills and regular resident progress monitoring.
  • Makes referrals, coordinates services, and consults with other providers, legal agencies, and community organizations.
  • Helps residents identify and link with community agencies, resources, and supports.
  • Coordinates educational and/or vocational skill development.
  • Organizes and maintains case records.
  • Assists with the admission process and orientation of new residents.
  • Ensures resident safety during assigned hours.
  • Responds to social or housekeeping issues as they arise.
  • Provides breathalyzer and toxicology screening as needed.
  • Responds to crises and emergencies.
  • Completes data input into the agency database.
  • Responds professionally to incidents involving contagious diseases.
  • Assists with medication self-administration policies and procedures.
  • Assists with transportation as needed.
  • Facilitates the resident government’s program guidelines.
  • Provides resident education.
  • Serves on agency committees as requested.
  • Carries out additional duties essential to the above-stated responsibilities as assigned.
  • Integrates Trauma-Informed Care principles in all duties and interactions.
  • Maintains compliance with Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid, HUD, and other required rules and regulations.
  • Maintains regular attendance at work location.

Requirements

  • Clinical Specialist I: Minimum HS Diploma or GED; Enrolled in 350-hour educational program or completion of one-time requirements within 3 months of hire; At least one year of experience in substance use, mental health, or human services field; Must possess a valid New York State Driver’s license and reliable automobile.
  • Clinical Specialist II: Minimum HS Diploma or GED; CASAC-Trainee or Provisional certification; Completion of a 350-hour educational program or 4000 hours of work experience; 85 hours of Section I education; Completion of OASAS one-time requirements; At least one year of experience in substance use, mental health, or human services field; Must possess a valid New York State Driver’s license and reliable automobile.
  • Clinical Specialist III: Minimum HS Diploma or GED; CASAC or other QHP status certification; At least one year of experience in substance use, mental health, or human services field; Must possess a valid New York State Driver’s license and reliable automobile.
