About the Role

The Patient Flow Coordinator - Referral Office (PFC) is responsible for managing internal and incoming (external) referrals to Medical Oncology and Hematology. The PFC manages the entire new patient intake process from initial referral to the first consult.

Responsibilities

  • Register patients and update provider information in the electronic patient record.
  • Organize referral documents and communicate with triaging physicians.
  • Coordinate the scheduling of patient appointments, including pulling patient clinic lists and reviewing booking calendars.
  • Answer patient queries and handle telephone activities.
  • Communicate effectively in person or by telephone with internal and external parties.
  • Perform cross-functional duties as assigned or requested.
  • Perform other duties consistent with the job classification.

Requirements

  • Completion of Grade XII or recognized equivalent.
  • Completion of Medical Office Administration Program.
  • Minimum of one (1) year related work experience.
  • Previous experience working in a Health care environment is preferred.
  • Previous experience working with the Electronic Patient Record (EPR) or Pathways Healthcare Scheduling (PHS) is preferred.
  • Working knowledge of computer software applications, preferably in a Microsoft Office environment.
  • Demonstrated commitment to client service and satisfaction.
  • Excellent interpersonal and listening skills.
  • Strong communication, organizational, time management, and problem-solving skills.
  • Strong judgment/decision-making skills.
  • Ability to remain calm and composed when dealing with difficult people or situations.
  • Ability to work efficiently in a high-pressured environment with simultaneous demands.
  • Adheres to confidentiality policy for sensitive patient data and information.
  • Demonstrated ability to work in a team environment and collaborate with others in assisting with the delivery of services.
  • Current UHN employees must have successfully completed their probationary period, have a good employee record, and satisfactory attendance to be eligible for consideration.
