About the Role

WellBe Senior Medical is changing how it feels to work in healthcare by replacing profit-driven volume with Value-Based Care delivered directly in the home. Join our award-winning team to leave burnout behind and practice medicine as it was intended: with autonomy, connection, and purpose.

Responsibilities

• Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home.
• Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients.
• Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
• Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.

Requirements

• Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.
• Active, unencumbered NP license and Board Certification (ANCC or AANP).
• Valid driver’s license and vehicle for local field-based travel.
• Applicants must be legally authorized to work in the United States without the need for employer sponsorship now or in the future.
• Successful completion of a pre-employment drug screening.
• Successful completion of a background check, as permitted by law.
• Satisfactory results of monthly FACIS checks.
