About the Role
People USA provides high-quality, effective peer services and behavioral health care to individuals with mental health and/or addiction issues. As a peer-run nonprofit, our agency is entirely composed of individuals with lived experience in recovery and wellness. Our mission is to empower people to manage and overcome behavioral health challenges. The Licensed Social Worker (LMSW)/Licensed Mental Health Counselor (LMHC) will oversee and deliver direct services, including assessments, treatment plans, and discharge planning, to participants at the Ulster County Crisis Support Center.
Responsibilities
- Provide oversight and direct services to individuals, including youth, and family therapeutic services.
- Conduct assessments, develop treatment plans, and manage discharge planning.
- Address referrals from community providers, including warm hand-offs.
- Engage in crisis de-escalation and therapeutic interventions.
- Lead daily huddles to assess needs and coordinate services.
- Collaborate with individuals, families, community providers, team members, and external partners.
- Complete and sign off on required documentation, including biopsychosocial assessments and treatment planning.
- Demonstrate cultural competency and a commitment to participants.
- Maintain a Trauma-Informed environment and a person-centered approach to care.
- Act as an advocate and assist individuals with resource linkages.
- Make psychosocial assessments and develop and implement care plans.
- Complete discharge planning and collaboration for continuity of care.
- Perform other assigned duties.
Requirements
- Ability to pass background checks.

- Knowledge of electronic medical records.

- Lived experience of mental health or substance abuse preferred.
- Working knowledge of Clinic policies and procedures.
