About the Role
Millennium Physician Group is seeking a full-time Case Manager to join the ACO team. We are looking for a positive, energetic, well-organized individual who can multitask and think outside the box, supporting our top initiative of ensuring an excellent patient experience.
Responsibilities
- Assist patients with achieving their highest level of wellness through direct provision of services.
- Focus on client-centered services linking patients and their families with health care and community-based services.
- Ensure timely, coordinated access to medically appropriate levels of health and support services.
- Maintain continuity of care.
- Perform assessment, planning, coordination, advocacy, education, and monitoring.
- Ensure accurate and timely documentation in all program areas.
- Demonstrate knowledge of growth and development.
- Assess, interact, communicate, and care for age-specific patient populations.
Requirements
- Registered Nurse with an active and clear license in the State of Florida.
- 3+ years of clinical experience.
- 1+ years of case management experience preferred.
- Strong computer skills, especially in Excel.
- Knowledge of EMR systems.
