About the Role

The Community Hospice is seeking a full-time Social Worker to join their Hospice Admissions Team. This role focuses on addressing the emotional, psychological, and social needs of patients and families navigating serious illness and end-of-life care, while collaborating with a multidisciplinary healthcare team.

Responsibilities

  • Provide psychosocial assessments and emotional support to patients and families.
  • Offer counseling and therapeutic interventions related to end-of-life issues, grief, loss, and adjustment.
  • Identify barriers impacting patient well-being and quality of life.
  • Connect patients and families with community resources and supportive services.
  • Advocate for patients and ensure care is provided in the most appropriate setting.
  • Collaborate with the interdisciplinary team to develop and implement patient-centered care plans.
  • Support staff through consultation and education related to psychosocial aspects of hospice care.

Requirements

  • NYS SW Licensure.
  • Valid NYS Drivers license.
