About the Role

The Counselor Assistant assists in the care of residents and provides support with the daily operation of the facility. The counselor assistant provides person-centered care coordination and case management to residents.

Responsibilities

  • Assists in resident care and supports daily facility operations.
  • Provides person-centered care coordination and case management.
  • Engages in resident daily activities, observes behavior, and ensures appropriate conduct.
  • Completes comprehensive service plans every 30 days in collaboration with clients.
  • Ensures the safety and integrity of the facility and its residents.
  • Secures records and monitors medication storage and access according to policy.
  • Upholds confidentiality regulations.
  • Provides referral and care coordination services to residents and communicates with service providers.
  • Delivers psychoeducation and training to residents in living skills.
  • Communicates effectively and cooperates with residents, staff, and professionals.
  • Documents according to reporting and recordkeeping policies.
  • Participates in the development and maintenance of the therapeutic community.
  • Provides basic crisis management and addresses concerns with appropriate staff and external providers.
  • Transports residents in agency vehicles for program-related activities.
  • Utilizes daily logs and progress notes to communicate shift information.
  • Provides screening, intake, and orientation services to residents.
  • Co-facilitates groups with CASAC level or higher staff, as assigned.
  • Meets one-on-one with residents for case management services and prepares weekly progress notes.
  • Provides comprehensive case management and facilitates access to community services.
  • Ensures compliant recordkeeping.
  • Actively participates in supervision and professional development/training.
  • Adheres to Agency policies and procedures.
  • Completes other duties as assigned.

Requirements

  • Minimum High School Diploma or GED required.
  • Associate’s degree in Human Service related field, or CASAC-T preferred.
  • Two years of prior experience in substance abuse or related human service field required.
  • Must be 21 years of age or older.
  • Relevant combination of education and experience will be considered.
  • Employee is frequently required to sit, talk, hear, stand, walk, use hands to type and/or perform light lifting.
  • Specific vision abilities include close vision, distance vision, and ability to adjust focus.
  • Adheres to all applicable federal and state laws and regulations.
  • All duties must be performed in accordance with CCFCS’s corporate compliance & ethics program.
  • Offer of employment contingent upon successful completion of a background check.
