About the Role

Catholic Charities of Orange, Sullivan & Ulster is seeking a full-time Case Manager for their Ulster County Case Management program to provide direct case management services to the needy and disenfranchised. This role involves advocacy, information and referral, crisis intervention, and short-term case management, as well as reception duties.

Responsibilities

  • Provide crisis intervention to homeless individuals and families facing housing emergencies.
  • Serve as a primary case manager for the New York State Rental Supplement Program Plan (RSP), Shelter Arrears Eviction Forestallment program (SAEF), and emergency relief program.
  • Conduct needs assessments and develop service plans using a short-term intervention model.
  • Conduct social care screenings of Medicaid members using the AHC Health-Related Social Needs Screening tool on Unite Us and refer cases as needed.
  • Navigate individuals not eligible for enhanced services to appropriate non-Social Care Network services.
  • Log all client interactions in Unite Us, Apricot, and/or other required systems.
  • Maintain accurate and complete work files, records, and documentation.
  • Process cases within a reasonable timeframe, preferably 45 days.
  • Answer telephone and provide information.
  • Conduct community outreach and perform data collection.
  • Assist with food/kids’ pantry distributions.
  • Participate in weekend and after-hours outreach activities, community tabling events, and special projects like Stuff-The-Bus, Adopt-A-Family, and Giving Tree.
  • Develop and maintain relationships with providers throughout Ulster County.
  • Attend community or service provider meetings.
  • Be available for planned weekend and some after-hours activities.
  • Participate in regular supervisory sessions.
  • Attend agency in-service training.
  • Travel to meet clients at other CCOSU offices.
  • Perform other duties as assigned.

Requirements

  • Must possess a clean NYS Driver’s License and maintain it throughout employment.

Skills

  • Knowledge of Social Services available in Ulster County
  • Computer literate with knowledge of Microsoft Word, Excel, and Outlook.
  • Excellent interpersonal and communication skills.
  • Good organizational, time-management, and communication skills.
  • Ability to work well independently as well as part of a team.
  • Ability to work with the public and/or under potentially stressful conditions.
  • Basic understanding of client engagement and de-escalation techniques.
  • Prepare comprehensive progress notes and service plans.
  • Remain stationary at a workstation and use a computer at least 30% of the time.
