About the Role
The Day Habilitation Coordinator/QIDP - Case Manager is responsible for developing and reviewing Day Habilitation Plans and Outcomes at the Day Community Opportunities Program. This role ensures that Outcomes align with the Day Habilitation Plan and acts as an advocate for enrolled individuals.
Responsibilities
- Develop and write Day Habilitation plan for each Participant.
- Rewrite the Day Habilitation plan to reflect changes.
- Coordinate and review the Participant’s plan with the family, Service Coordinator, and Participant on a semi-annual basis to discuss progress/regression and overall performance within Day Habilitation.
- Communicates with the Participant and/or provider to ensure that the Day Habilitation plan of service meets the Participant’s needs and desires; supplies guidance or direct intervention when it does not.
Requirements
- Bachelor’s Degree in human services.
- One (1) year of experience in providing services to persons with developmental disabilities.
- Good writing, typing and interpersonal communication skills.
- Valid NYS driver’s license and reliable transportation.
