About the Role
Sun River Health is seeking a full-time Unit Supervisor of Health Home Care Management to supervise care management staff and oversee daily operations, ensuring quality of care and program compliance.
Responsibilities
- Ensure coverage and provide care coordination for assigned care manager caseloads and/or cover caseloads as needed
- Provide crisis intervention to clients in psychological distress; review and approve client documentation as assigned
- Review assessments, plans, progress notes and other documentation as required to ensure necessary detail, content to meet regulatory requirements and standards
- Develop and Implement quality assurance and quality improvement activities
- Consult with multidisciplinary team on client’s care plan/needs/goals, including case conferencing, family members, and social supports
- Provide supervision and support to assigned staff, including hiring, scheduling, evaluating, staff development, team meetings, and individual supervisory meetings
Requirements
- Bachelor's Degree required
