About the Role

Horizon Health Services is seeking dedicated individuals to join their team as counselors. Responsibilities

  • Serve as the initial clinical contact for patients beginning their wellness and recovery journey.
  • Conduct short-term, person-centered assessments for case conceptualization and patient-counselor matching.
  • Utilize engagement skills with patients, families, and support systems to establish the treatment process and improve outcomes.
  • Receive training on evidence-based treatment approaches and ongoing learning opportunities with CEUs.
  • Collaborate with a multidisciplinary team, receiving ongoing coaching and clinical support.
  • Develop mentoring and leadership skills through group supervisions, case conferences, and 1:1 feedback.
  • Build collaborative relationships across the agency during the transition of care process by making clinical recommendations.
  • Function as a therapist within a comprehensive system offering care coordination, psychiatric services, peer supports, financial social work, and crisis management.

Requirements

  • CASAC-T (Credentialed Alcohol and Substance Abuse Counselor) Certification or higher plus a Bachelor’s Degree
  • Completed Master’s degree in a program that will grant licensure in the state of New York
  • Ability to be available at least two evenings per week.
  • Physical ability to sit for extended periods, alternate between standing and sitting, perform repetitive wrist, hand, and finger movements, and occasionally lift up to 20 pounds.
