About the Role

As a member of the interdisciplinary care team in this multi-specialty care setting, the social worker will support the following medical specialty practices: Clinical Immunology, Endocrinology, Gastroenterology, Infectious Diseases, Renal, and Sleep Medicine.

Responsibilities

• Collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention.
• Support medical specialty practices including Clinical Immunology, Endocrinology, Gastroenterology, Infectious Diseases, Renal, and Sleep Medicine.

Requirements

• Must be a licensed social worker.
• Employment provides a pathway to LCSW licensure.
