About the Role

Metro Physical & Aquatic Therapy is seeking motivated Physical Therapists for home-based care, offering flexibility, autonomy, and meaningful patient relationships. Our clinicians focus on helping patients maintain mobility, independence, and overall quality of life in their homes and communities.

Responsibilities

  • Accept and treat patients within your designated geographic area
  • Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
  • Develop a plan of care and treatment goals in consultation with the physician, patient, and family
  • Plan, prepare and implement customized therapy programs to restore function and promote independence
  • Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
  • Provide timely and accurate documentation for each visit
  • Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
  • Deliver care in compliance with agency policies and professional practice standards

Requirements

  • Physical Therapist Program Graduate
  • Licensed Physical Therapist in the state of New York or eligible
  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
  • Ability to travel to patients’ homes within the assigned geographic territory

Location

  • In and around Hempstead

Work Type

Salary/Compensations

  • $70-$90 per visit
