About the Role

Licensed Practical Nurses at the Long Island State Veterans Home are critical team members who diagnose and treat health responses, providing supportive and restorative care while adhering to medical regimens.

Responsibilities

  • Provides and ensures nursing care to residents within the medical and nursing care plan, and under the supervision of a professional nurse.
  • Monitors residents for response to treatments, medication, and changes in health status.
  • Prepares and dispenses medication according to the medical plan of care.
  • Assists residents with activities of daily living, observes status and changes, and supervises non-professional staff.
  • Documents resident progress notes, and assists with weekly, monthly, and annual documentation.
  • Communicates changes and incidents to supervisor and completes required documentation.
  • Maintains policies and procedures relevant to medications, including management of controlled substances.
  • Communicates pertinent information and changes impacting resident care to supervisor, nursing staff, and other departments.
  • Listens and responds appropriately to concerns expressed by residents and families.
  • Ensures direct care is consistent with comprehensive assessment, resident requests, and plan of care goals.
  • Works closely with other professionals and disciplines to ensure timely and coordinated care.
  • Participates in the Department's Quality Assurance Program and serves on relevant committees.
  • Participates in the orientation of new employees and clinical in-service of non-professional staff.

Requirements

  • NYS LPN License.
  • Successfully clear a background investigation.
  • Successfully complete pre-employment physical examination and obtain medical clearance.
  • Complete electronic reference check with a minimum of three (3) professional references.
  • Provide a copy of the required license(s)/certificate(s).
