About the Role

The Behavioral Health Licensed Practical Nurse (LPN) will function as a member of the Integrated Health Care Behavioral Health Prescriber Services Nursing Team. The LPN will assist the Behavioral Health Psychiatric Prescribers in their provision of care while providing a full range of nursing and medical support services for clients enrolled in IHC’s Article 31 and Article 32 clinics.

Responsibilities

  • Manage the Nursing Triage and Medication Line, responding to patient triage and support calls, and coordinating care as needed.
  • Coordinate the Prescriber Services Bridge Script Process, assisting in providing medication refills, prior authorizations, and tasks related to ensuring prescribed medication regimes are made available to clients.
  • Manage the Initial Psychiatric Evaluation Process, including coordinating receipt of records, blood work and EKG results, and necessary clinical information from the client’s care team.
  • Utilize the electronic health record, referral platforms, and other IT resources to facilitate efficient management of administrative functions.
  • Maintain detailed documentation, spreadsheets, and workflows to effectively complete job duties in an organized and detail-oriented manner.
  • Administer Long-Acting Injection Medications when necessary due to RN absence, when ordered by a clinic prescriber and using best practices.
  • Facilitate Health Monitoring to address health implications and support recovery when necessary due to RN absence.
  • Respond to staff/client mental and physical health emergencies/crises at the clinic.
  • Provide assessments, consultations, and clinic-based services for clients and/or staff to determine current health needs.
  • Monitor follow-up care via review of medical records, lab test results, and contact with health providers, as needed.

Requirements

  • Graduate of an accredited LPN program with current licensure in NYS.
  • One year of clinical experience as an LPN.
  • Must have eligibility for full and unconditional participation in the Medicaid and Medicare programs.
  • Excellent verbal and written communication skills.
  • Ability to effectively use an electronic medical record for all documentation.
