About the Role

United Health Services (UHS) is seeking a Discharge Planner to join our Comprehensive Psychiatric Emergency Program (CPEP). This role supports patients during critical transitions of care, connecting them with resources for continued treatment and recovery. You will collaborate with a multidisciplinary team to coordinate safe, timely, and patient-centered discharge plans for individuals experiencing psychiatric and behavioral health emergencies.

Responsibilities

  • Supporting patients during critical transitions of care.
  • Connecting patients with resources needed for continued treatment and recovery.
  • Collaborating closely with Nurse Care Managers, behavioral health professionals, patients, families, and community partners.
  • Coordinating safe, timely, and patient-centered discharge plans for individuals experiencing psychiatric and behavioral health emergencies.

Requirements

  • Associate in applied science degree in a related field or five (5) years of experience in an acute/tertiary facility and/or community health nursing.

Location

  • UHS Binghamton General Hospital

Work Type

  • Day Shift – 32 hours per week
  • Evening Shift – 32 hours per week

Education Level

  • Associate in applied science degree in related field
  • Bachelor's of Science degree in health care or related field

Salary/Compensations

  • $27.80 - $41.69 per hour, depending on experience
