About the Role

The Mental Health Skill Building Specialist assists individuals with overcoming mental health barriers that may interfere with independent functioning and performing normative adult roles in the community. This role involves helping clients acquire, retain, and improve life skills such as communication, self-advocacy, socialization, and community resource utilization.

Responsibilities

  • Assist clients in identifying and achieving goals through a person-centered assessment process.
  • Provide one-to-one supportive services on-site and in the community.
  • Complete necessary documentation, including assessments, progress notes, and care plans.
  • Function as a member of an interdisciplinary team and contribute to comprehensive treatment plans.
  • Assist with the collection of Quality of Care monitoring.
  • Provide recovery-oriented activities to restore or build social and interpersonal skills.
  • Assist individuals with advocating for their needs with healthcare providers.
  • Support relapse prevention planning, chronic condition management, and medication management.
  • Establish good health routines and practices.
  • Provide referrals for needed services and collaborate as appropriate.
  • Complete written reports, statistics, case files, and summaries in a timely manner.
  • Assume administrative tasks delegated by the Program Supervisor.
  • Use personal vehicle to travel to and from community appointments.
  • Perform other duties, responsibilities, and related tasks as assigned.

Requirements

  • HS diploma or equivalent and at least one to three years of relevant experience working with individuals with Substance Use Disorders and/or Serious Mental Illness, OR a Bachelor's degree.
  • Strong organizational, engagement, and interpersonal skills.
  • Ability to work independently and with flexibility in days and hours of work.
  • Demonstrates sound judgment.
  • Ability to utilize critical thinking and problem-solving skills.
  • Valid NYS Driver's license.
  • Access to reliable transportation.
