About the Role
The primary role of the Adult Protective Case Manager is to manage services for individuals who are unable to meet their essential needs, secure entitlements, or protect themselves from harm, and have no one readily available to assist them. This includes referrals to the Rep Payee Program and other services as appropriate.
Responsibilities
- Conduct face-to-face and home visits to assess client safety, risk, and needs at least monthly.
- Screen program referrals from OCDFCS on the date of referral.
- Meet with OCDFCS staff and client within three days of referral.
- Determine and assist clients in meeting their day-to-day living needs (food, clothing, housing, medical care, social supports).
- Meet with OCDFCS monthly to discuss client status and progress.
- Provide monthly status reports of all clients, including financial and personal details.
- Maintain accurate client files and provide progress notes to OCDFCS within ten days of an event.
- Contact OCDFCS of any changes in client's personal situation requiring intervention.
- Maintain an accurate mileage log for home visits.
- Provide clients with networking resources for additional services.
- Report critical client concerns to Program Manager and/or administration staff promptly.
- Prepare Social Security or other assigned reports specific to client caseload.
- Prepare additional paperwork for networked agencies as necessary.
- Meet with Rep-Payee Program Manager weekly to discuss client issues and updates.
- Attend and participate in departmental and staff meetings as requested.
- Perform other duties as designated by supervisor.
Requirements
- Bachelor’s degree in Human Services, Social Work, Sociology, Psychology, Child Development or equivalent required.
- Minimum of three years of experience in Human Services with a public or private agency.
- Experience with one-to-one client interaction, identifying needs and goals for service plan development.
- Intermediate skills with Microsoft Word, Excel, and Outlook.
- Possession of a valid NYS driver’s license and own transportation required.
- Enthusiastic support of the Mission Statement.
