About the Role
Family Service League is seeking a full-time Case Manager for the Alternatives to Youth Program. This role will coordinate in-home family support to divert involvement in the Suffolk County Probation System, focusing on empowering parents and equipping families for success. The Case Manager will conduct client home visits and provide direct services to youth.
Responsibilities
- Provide linkages, coordination, advocacy, and mentoring.
- Engage families in conflict management and skill building.
- Provide direct services, including anger management and parenting skills to youth and parents, to avoid involvement with the juvenile justice system.
- Conduct assessments and maintain electronic health records of all services provided.
- Conduct home visits throughout Suffolk County.
- Perform other duties as assigned.
Requirements
- Bachelor's degree with one year of human services experience, or Associate's degree with three years of human service experience.
- At least one year of experience working with community resources.
- Excellent interpersonal, organizational, and problem-solving skills.
- Strong communication skills, verbal and written.
- Ability to work as part of a team.
- Proficient computer skills, including Microsoft Office.
- Valid and clean NYS Driver's license.
- Ability to travel.
