About the Role

The Community Habilitation Skill Developer provides training, instruction, and assistance to individuals with mental retardation and developmental disabilities in their homes and communities, aiming to achieve valued outcomes outlined in their Community Habilitation Plan.

Responsibilities

• Implement the Community Habilitation Plan by providing instruction related to individualized valued outcomes.
• Document individuals' valued outcomes on billing documents contemporaneously.
• Submit monthly progress notes on individuals' response to valued outcomes.
• Ensure consumer health, safety, and welfare at all times.
• Report unusual occurrences or situations to the Supervisor immediately.
• Submit weekly parent/consumer verification forms to confirm service delivery.
• Attend all scheduled meetings and in-services.
• Submit bi-weekly timesheets to ensure timely staff payment.
• Report to work as scheduled and communicate any changes in a timely manner.
