About the Role
CMCS (Cardinal McCloskey Community Services) is seeking a dedicated and compassionate LPN Nurse Care Manager to join our Special Medical Preventive Services Program. This role supports the health and well-being of children and families with complex medical needs, ensuring they receive coordinated, high-quality care and guidance. This position is ideal for an LPN passionate about community health, hands-on family engagement, and collaborative environments.
Responsibilities
- Assist in addressing the health needs of families enrolled in the Special Medical Preventive Services (SMD) Program.
- Deliver services in accordance with ACS, OCFS, state, and agency mandates.
- Conduct family-based home visits to assess health concerns and provide education and support.
- Participate in group health education activities aimed at empowering families.
- Serve as a medical liaison between families, hospitals, clinics, and other community healthcare providers.
- Ensure seamless communication and coordination of all family health services.
- Provide guidance in preventive, curative, and rehabilitative health matters.
- Assist families in understanding medical recommendations, follow-up care, and management of chronic or communicable conditions.
- Contribute to the development of community health nursing plans addressing maternal and child health, chronic illnesses, disease prevention, and family health education.
- Participate in referral and follow-up processes for infants, children, adolescents, and adults.
- Work closely with social workers, program leadership, and medical providers to ensure comprehensive, family-centered care.
Requirements
- Bilingual Preferred.
- Licensed Practical Nurse with current New York State Licensure.
- Previous experience providing health care management / community-based nursing.
