About the Role

St. Ann’s Community is seeking an LPN Dementia Services Coordinator for Rainier Grove, an assisted living memory care center on our Webster campus. This role supervises resident care, case management, medication assistance, and overall care provided by LPNs, Resident Care Aides, and other staff to meet or exceed NYSDOH requirements.

Responsibilities

  • Ensures staff provide residents with quality personal care services, enhancing quality of life and promoting the highest level of functioning.
  • Maintains resident charts and organizes the medication room.
  • Assists with documentation for resident admissions, transfers, and discharges.
  • Supports the Case Manager in developing, implementing, and reviewing individualized resident care plans.
  • Coordinates medical, nursing, and psychosocial services for residents.
  • Provides supervision and assistance with personal care needs, including toileting, bathing, dressing, grooming, and mobility programs.
  • Supervises resident medication administration and assists with prescribed treatments.
  • Maintains accurate inventory of narcotics and completes shift documentation.
  • Observes, reports, and documents changes in resident health and behavior.
  • Contacts resident responsible parties to report changes in condition or falls.
  • Reports resident incidents to the Assisted Living Administrator and completes incident reports.
  • Provides direction, education, and development to nursing staff and Resident Care Aides.
  • Participates in staff selection and hiring.
  • Conducts orientation for new staff.
  • Evaluates health-related interventions.
  • Develops plans for ongoing in-service education for staff.

Requirements

  • LPN License Required
  • Prior supervisory experience needed
  • Experience in a dementia-related health setting
  • Experience with electronic health records
