About the Role

As a community health worker (CHW), you’ll have an important role in creating a positive experience for patients at Neighborhood. You’ll listen with kindness to patients’ concerns, seek solutions, and assist patients and families with navigating services and community resources.

Responsibilities

  • Consult with providers regarding patient needs for referrals or linkages and assist patients in accessing and completing referrals.
  • Assist patients in accessing community resources such as food pantries, clothing, shelter, transportation, education, parenting or other services as referred by providers.
  • Assist patients in completing healthcare forms such as applications for Medicaid, Child and Family Health Plus and Sliding Fee Scale program.
  • Connect patients to facilitated enrollers.
  • Obtain prior authorizations as needed.
  • Document and track referrals until completion.
  • Conduct community outreach to identify people who are experiencing homelessness and educate them about health needs.
  • Support people who are experiencing homelessness in accessing healthcare services at Neighborhood.
  • Serve as health and community resources coordinator for patients and providers.
  • Advocate for patients with community agencies and services.
  • Conduct outreach and speak to community groups.
  • Assist in developing Neighborhood staff to improve patient navigation, decrease barriers for patients in accessing care and connect patients to care.

Requirements

  • Experience in community health, patient resource coordination or equivalent experience
  • Medical office and medical terminology knowledge and experience preferred
  • Attention to detail and follow-through, organizational and influencing skills
  • Experience with Electronic Health Records (EHRs), Microsoft Office Suite, and other computer software
  • Excellent oral and written communication skills

  • Teamwork: be open to others’ unique perspectives, and collaborate to meet shared goals
  • Must be available to work Monday-Friday between 8:30 a.m. and 4:30 p.m.
  • Ability to travel for community outreach
  • Ability to work extended hours once a week 7:00-9:00 p.m.
