Responsibilities

  • Provide case management to assist clients in overcoming barriers to maintaining housing and family stabilization using a Family Development strengths-based approach.
  • Triage caseload to ensure adequate support for households in crisis and those with high needs.
  • Complete comprehensive assessments of household strengths and needs using agency tools and best practices.
  • Develop goals and milestones with household participation according to the Brief Solution Focused treatment model and Family Development.
  • Conduct regular evaluations of progress and modify goals periodically to support successful retention of permanent housing.
  • Communicate and work effectively with a team, both in-house and through case conferencing.
  • Ensure comprehensive access to all SCAP services by working with agency colleagues and community partners to integrate, streamline, and coordinate services.
  • Conduct advocacy on behalf of households in a solution-focused manner and represent SCAP professionally.
  • Conduct weekly home visits and interact with clients at off-site locations to observe and support task completion, crisis amelioration, and housing stability.

Requirements

  • Bachelor’s Degree in social work, human services or related field; Master’s Degree preferred.
  • Two years of experience providing related services to families and individuals; one year with a Master’s Degree.
  • Knowledge of community-based services, public assistance, and other community resources, with demonstrated skills in accessing these services appropriately.
  • Ability to quickly learn available resources and appropriately refer clients to ensure full access to SCAP’s services.
  • Demonstrated written and verbal communication skills and ability to employ appropriate boundaries, representing SCAP professionally.
  • Ability to perform job responsibilities independently.
  • Valid New York State driver’s license, regular access to a car, and willingness to travel to off-site locations.
  • Demonstrated computer expertise and ability to stay up-to-date on software used in best practices.
  • Background check required through the NYS Central Registry of Child Abuse.
  • Knowledge and understanding of issues related to individuals and families in poverty.
  • Ability to build upon the strengths of diverse individuals and families to provide high-level services and lasting solutions.
