About the Role

The Case Manager coordinates, manages, documents, and supports the treatment and programming of youth. This role also serves as a liaison, developing and maintaining rapport and communication with the county, family, and youth. It is a full-time, benefit-eligible position with flexible scheduling, occasional overtime, and requires use of a personal vehicle for business purposes.

Responsibilities

  • Collaborates and communicates with county workers, service providers, program staff, and family members to advocate for a youth’s best interests.
  • Communicates and consults regularly with the Clinician regarding treatment implications and goals.
  • Collaborates with internal departments to ensure service and treatment congruency.
  • Attends all required meetings.
  • Maintains all required record keeping, reports, and documentation.
  • Provides direct support to families, including visitations and home assessments.
  • Collaborates with the program management team on treatment, program planning, paperwork, and intra-agency communication.
  • Represents the organization professionally, upholding its values, mission, and philosophy.
  • Demonstrates and promotes principles of Therapeutic Crisis Intervention (TCI), Boys Town Psychoeducational Model (PEM), and other skills-based models.
  • Understands and adheres to all Agency policies, protocols, and procedures.
  • Manages paperwork demands within established time frames.
  • Maintains productive and professional relationships with staff, youth, parents, and community partners.
  • Maintains the highest ethical standards and conduct.
  • Ensures regular and timely attendance.

Requirements

  • Ability to work independently and collaborate effectively within a multidisciplinary team.
  • Ability to work independently and without direct oversight.
  • Ability and willingness to take initiative.
  • Strengths-based and client-centered approach.
  • Experience and/or familiarity with mental health related community-based supports.
  • Ability to lift 25 pounds without assistance.
  • Ability to sit, squat, crouch, and stand for elongated periods of time.
  • Ability to lift arms over head in an upward reaching motion.
  • Ability to walk up and downstairs up to several times per day.
  • Ability to use systems including Word, Excel, Outlook, and Teams.
  • Valid driver’s license and auto insurance, free from major infractions and deemed acceptable to the insurance carrier.
  • Varying levels of travel may be required.
