About the Role
We are a physician-led, patient-centric network dedicated to simplifying healthcare and providing connected care. This role involves coordinating patient care, facilitating communication between patients and providers, and delivering direct clinical support within our multispecialty and urgent care practices.
Responsibilities
- Coordinate daily patient care and physician access
- Act as a liaison between patients, staff, and physicians
- Maintain efficient patient flow
- Deliver direct patient care within scope of licensure and company policy
- Collect and accurately label lab specimens
- Implement physician orders and nursing procedures
- Prepare for patient visits by gathering pertinent information
- Communicate issues promptly to management
- Verify patient identity prior to care or documentation
- Utilize EHR tasking effectively
- Obtain and document vital signs including blood pressure, temperature, pulse, height, and weight
- Administer injections and document accurately in EHR
- Perform telephone triage and document in EHR
- Schedule appointments according to department guidelines
- Adhere to universal safety precautions for medication administration and medical waste disposal
- Review and update medication lists and document allergies in EHR
- Manage prescription refills and control procedures
- Enter orders and manage referrals in Practice Management System and EHR
- Maintain emergency equipment logs
- Facilitate transitions of care to urgent care or hospital settings
- Maintain and troubleshoot department-specific equipment
