About the Role
Assists in the implementation of the person's ISP/Day Habilitation Plan by planning, providing and documenting community integration experiences utilizing community-based resources and supports. Assists in providing direct care, training, and supervision as required.
Responsibilities
- Ensures supervision of assigned individuals at all times.
- Provides transportation for community outings, following established policies and procedures for agency vehicle use.
- Promotes and follows person-driven choices in independence, individualization, inclusion, and productivity, adhering to the 'Five Governing Principles'.
- Provides necessary assistance with activities of daily living, including toileting, eating, ambulation, and lifting.
Requirements
- Associate Degree in human services and experience/education in the Fine Arts field.
- High school diploma or GED and 1 year of experience working with persons with developmental disabilities, in an educational setting, or in a healthcare facility.
- Ability to transfer and position individuals and manipulate equipment such as wheelchairs.
- Valid NYS Driver's License.
