About the Role
Springboard's Transitional Aged Youth (TAY) Home is a supported living program designed to assist transitional aged youth with developmental disabilities in their transition from the child welfare system. The program focuses on developing essential life skills, fostering independence, and creating a supportive environment where individuals can achieve their personal goals.
Responsibilities
- Collaborate with individuals to develop goals, plans, and strategies for independent living.
- Conduct needs analyses to identify individual requirements.
- Support the delivery of services and supports aligned with goal plans for independent living.
- Locate programs, services, housing, and employment opportunities for individuals.
- Work with staff to develop strategies and implement therapeutic programming.
- Provide direction to Junior Developmental Service Workers on program delivery and daily living activities.
- Ensure effective, trauma-informed personal and physical care, crisis response, counseling, and emotional support.
- Support program development within and outside the home, facilitating life skills for independent community integration.
- Ensure program plans promote independent living, are needs-based, reflect high learning, and are developed collaboratively with the Supervisor.
- Facilitate life skills programming (one-on-one or group) based on individual needs, goals, and support plans.
- Participate in assessing long-term service needs for individuals and the program.
- Provide support for individuals to safely participate in community activities and recreational outings.
- Plan community outings, appointments, and activities considering individual needs, safety, resources, and programming.
- Reduce safety incidents through positive behavior reinforcement and by identifying pre-, during-, and post-behavior patterns.
- Apply Individual Behavior Support Plans and CPI consistently during crises or challenging behaviors.
- Ensure written materials and correspondence are professional, clear, and reflect agency philosophy and privacy standards.
- Monitor finances for individuals as required by policies.
- Support staff scheduling as needed.
Requirements
- Post-secondary diploma in Social Services and/or Developmental Services, or a diploma in a non-related area with two years of experience in developmental services.
- Valid First Aid and CPI certifications.
- Strong knowledge of computer applications including MS Word, Excel, PowerPoint, and Outlook.
- Minimum two years of recent work-related experience in working directly with individuals with a developmental disability or dual diagnosis.
- Experience with case management.
- Experience supporting youth involved in the child welfare system.
- Experience working from a trauma-informed and harm reduction approach.
- Direct experience engaging with diverse populations and the ability to apply a culturally responsive lens.
- Understanding of the Social Inclusion Act and the Act for the Supports of Persons with Developmental Disabilities (2008).
- Working knowledge of the Developmental Services Sector.
- Employment requires satisfactory Vulnerable Sector Check (VSS), references, and proof of education.
