About the Role

The Licensed Practical Nurse (LPN) performs a variety of duties and responsibilities with an emphasis on quality of care and customer service. They provide services directly and collaborate with providers and outside agencies to ensure a patient-centered approach to care.

Responsibilities

  • Collaborates with and supports team members to ensure a patient-focused approach to care
  • Provides outstanding customer service while maintaining a compassionate, positive, and respectful attitude toward people who use drugs
  • Assesses, plans, implements and evaluates care for patients under their scope of practice
  • Prepares patients for provider visits by taking vital signs, history and assessing risk factors
  • Collects and records health information findings of physical and developmental assessments into patient record with timeliness and accuracy
  • Accesses and retrieves medical records via HEALTHeLINK™
  • Responsible for screening all new and returning patients by completing an intake process
  • Comprehensively documents intake encounters and health history into our EMR system
  • Facilitates lab and diagnostic tests such as injections, urine and oral toxicology’s at the request of the provider

Requirements

  • Current LPN license
  • Current BCLS certification
  • Minimum of one (1) year of experience working in a medical practice setting
  • Prior phlebotomy experience required
  • Ability to communicate clearly and professionally in writing and verbally
  • Familiarity with computers and MS Office software (Outlook, Word)
  • Familiarity with electronic charting/medical records systems (Medent preferred)
  • Demonstrate personal responsibility and integrity
  • Ability to handle Protected Health Information (PHI) in a manner consistent with HIPAA
  • Awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential
