About the Role
The Licensed Practical Nurse (LPN) performs a variety of duties and responsibilities with an emphasis on quality of care and customer service. They provide services directly and collaborate with providers and outside agencies to ensure a patient-centered approach to care.
Responsibilities
- Collaborates with and supports team members to ensure a patient-focused approach to care
- Provides outstanding customer service while maintaining a compassionate, positive, and respectful attitude toward people who use drugs
- Assesses, plans, implements and evaluates care for patients under their scope of practice
- Prepares patients for provider visits by taking vital signs, history and assessing risk factors
- Collects and records health information findings of physical and developmental assessments into patient record with timeliness and accuracy
- Accesses and retrieves medical records via HEALTHeLINK™
- Responsible for screening all new and returning patients by completing an intake process
- Comprehensively documents intake encounters and health history into our EMR system
- Facilitates lab and diagnostic tests such as injections, urine and oral toxicology’s at the request of the provider
Requirements
- Current LPN license
- Current BCLS certification
- Minimum of one (1) year of experience working in a medical practice setting
- Prior phlebotomy experience required
- Ability to communicate clearly and professionally in writing and verbally
- Familiarity with computers and MS Office software (Outlook, Word)
- Familiarity with electronic charting/medical records systems (Medent preferred)
- Demonstrate personal responsibility and integrity
- Ability to handle Protected Health Information (PHI) in a manner consistent with HIPAA
- Awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential
