About the Role

The Home Support Supervisor leads a team of Personal Support Workers in the field, providing ongoing training, coaching, and performance management to ensure high-quality home care delivery.

Responsibilities

  • Lead and manage a team of Personal Support Workers in the field
  • Provide ongoing training, coaching, and performance management
  • Travel to patient homes within the community
  • Collaborate with multidisciplinary teams in a community setting
  • Assess patient needs and formulate care plans
  • Monitor patient progress towards outcomes
  • Conduct performance appraisals and implement disciplinary actions
  • Analyze patient and employee feedback
  • Track key indicators including caseload volume, staff turnover, and satisfaction levels
  • Address and document patient concerns and complaints in a timely manner

Requirements

  • Current registration with the College of Nurses of Ontario in good standing
  • Minimum three years of experience with progressive leadership responsibilities
  • Valid driver's license and access to a reliable vehicle
  • Ability to write reports and communicate effectively
  • Excellent English verbal and written communication skills
