About the Role
The Home Support Supervisor leads a team of Personal Support Workers in the field, providing ongoing training, coaching, and performance management to ensure high-quality home care delivery.
Responsibilities
- Lead and manage a team of Personal Support Workers in the field
- Provide ongoing training, coaching, and performance management
- Travel to patient homes within the community
- Collaborate with multidisciplinary teams in a community setting
- Assess patient needs and formulate care plans
- Monitor patient progress towards outcomes
- Conduct performance appraisals and implement disciplinary actions
- Analyze patient and employee feedback
- Track key indicators including caseload volume, staff turnover, and satisfaction levels
- Address and document patient concerns and complaints in a timely manner
Requirements
- Current registration with the College of Nurses of Ontario in good standing
- Minimum three years of experience with progressive leadership responsibilities
- Valid driver's license and access to a reliable vehicle
- Ability to write reports and communicate effectively
- Excellent English verbal and written communication skills
