About the Role
High-Fidelity Wraparound is an evidence-based, intensive care coordination model designed to support children and youth with serious behavioral health challenges in living successfully within their communities and families. The Care Manager will assist in developing a cohesive Child and Family Team to create and implement a Wraparound Plan tailored to the youth and family's needs, strengths, cultural values, and vision.
Responsibilities
- Assist in developing a cohesive Child and Family Team composed of healthcare, social care needs providers, school personnel, and the family’s natural supports.
- Ensure the High-Fidelity Wraparound model is implemented with fidelity for each member and family.
- Carry out all elements of the Children’s Health Home Care Management (Community Outreach & Care Coordination) program.
- Transport clients to community resources and meetings.
Requirements
- Minimum of 2 years’ experience in the human services field (child welfare, mental health, substance abuse and physical health).
- Local travel is required; must be able to meet minimum agency insurance requirements for authorized drivers (minimum age 21).
- Must have a clean and valid NYS driver’s license, actively licensed for at least one year.
- Must have a reliable, fully insured vehicle for transporting clients.
- Ability to work flexible hours to accommodate client schedules, including evenings.
- Must possess the ability to make sound, independent decisions.
- Ability to pass the test to become a certified CANS assessor.
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and Google Business Workspace Suite (Gmail, Google Docs, Meet, Sheets, etc.).
- Must be willing and able to work in close proximity with clients.
- Must successfully clear pre-employment background checks, including fingerprinting.
