About the Role

Serves as an advocate for each resident, ensuring all needs are met and rights are protected. Provides supervision, assistance, and training to Direct Care staff.

Responsibilities

  • Assigns Residence Counselor and ensures Advocate responsibilities are met.
  • Coordinates healthcare and appointments for individuals, providing transportation as needed.
  • Ensures physician's and treatment personnel's orders are followed.
  • Coordinates medication administration according to agency policy.
  • Serves as on-site supervisor for individual financial accounts and spending, managing personal income.
  • Maintains up-to-date records for each individual, including various reports and plans.
  • Participates in the planning and development of each individual’s personal futures plan.
  • Ensures emergency backup staff coverage.
  • Maintains staffing schedules.
  • Coordinates time off authorization and verifies timesheets.
  • Conducts quality assurance reviews of systems and staff.
  • Provides performance appraisals and ensures staff conformity to services rendered.
  • Coordinates and ensures all required training.
  • Screens and interviews applicants for residence positions, submitting recommendations.
  • Plans and facilitates team meetings with Service Coordinator for effective service delivery.
  • Participates in all individual movements, including referrals, admissions, placement, discharge, and fund re-determination.
  • Ensures home maintenance is complete, coordinating with the Maintenance Department or securing outside services.
  • Ensures individuals' nurturing needs are met, including grocery shopping and menu planning.
  • Performs or assures household tasks such as meal preparation, cleaning, laundry, vehicle maintenance, clothing repair, and shopping.
  • Acts as administrator on duty for the Residential Program on a rotating basis.

Requirements

  • Must possess a valid NYS driver’s license.
  • Must have a reliable vehicle to transport service recipients.
  • Must have the ability to read, analyze, present information to groups, and solve practical problems.
  • Adhere to all CDS Monarch policies and procedures.
  • Adhere to the Agency Mission, Vision, Shared Values, and Customer Service Standards.
  • Attend mandatory Education and Training modules as scheduled, obtains, and maintains required certifications.
  • Act as a professional representative of CDS Monarch in regards to appearance, behavior, temperament, communication, language, and dress.
  • Perform all other duties relevant to the position as assigned by supervisor.
