About the Role
Unity House of Troy's ACT Team is seeking a compassionate Registered Nurse to provide person-centered, recovery-oriented care in the community. You will collaborate with a multidisciplinary team to promote wellness and independence for individuals living with serious mental illness.
Responsibilities
- Serve as a member of a multidisciplinary Assertive Community Treatment (ACT) team.
- Conduct comprehensive mental health and nursing assessments.
- Assess participants' physical health needs and provide ongoing nursing support.
- Coordinate referrals and communication with primary care providers and community physicians.
- Administer and monitor medications in collaboration with the ACT psychiatrist.
- Provide medication education and support to promote adherence and recovery.
- Deliver health education focused on wellness, chronic disease management, and self-management.
- Monitor participants for changes in mental and physical health status and respond appropriately.
- Participate in treatment planning and coordinate care with internal and external providers.
- Maintain accurate, timely, and compliant clinical documentation.
- Provide rehabilitation, recovery, and support services in community-based settings.
- Promote a culture of inclusion, belonging, dignity, and respect.
- Perform other duties as assigned.
Requirements
- Current, unrestricted New York State Registered Nurse (RN) license required.
- Nurse Practitioners are encouraged to apply (must possess prescriptive authority and a current DEA registration).
- Minimum of one (1) year of nursing experience in a mental health, behavioral health, or substance use treatment setting preferred.
- Knowledge of psychiatric medications, recovery-oriented care, and integrated behavioral healthcare.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Strong assessment, communication, and organizational skills.
- Proficiency with electronic health records (EHR) and Microsoft Office applications.
- Valid New York State Driver License with an acceptable driving record.
