About the Role

Unity House of Troy's ACT Team is seeking a compassionate Registered Nurse to provide person-centered, recovery-oriented care in the community. You will collaborate with a multidisciplinary team to promote wellness and independence for individuals living with serious mental illness.

Responsibilities

  • Serve as a member of a multidisciplinary Assertive Community Treatment (ACT) team.
  • Conduct comprehensive mental health and nursing assessments.
  • Assess participants' physical health needs and provide ongoing nursing support.
  • Coordinate referrals and communication with primary care providers and community physicians.
  • Administer and monitor medications in collaboration with the ACT psychiatrist.
  • Provide medication education and support to promote adherence and recovery.
  • Deliver health education focused on wellness, chronic disease management, and self-management.
  • Monitor participants for changes in mental and physical health status and respond appropriately.
  • Participate in treatment planning and coordinate care with internal and external providers.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Provide rehabilitation, recovery, and support services in community-based settings.
  • Promote a culture of inclusion, belonging, dignity, and respect.
  • Perform other duties as assigned.

Requirements

  • Current, unrestricted New York State Registered Nurse (RN) license required.
  • Nurse Practitioners are encouraged to apply (must possess prescriptive authority and a current DEA registration).
  • Minimum of one (1) year of nursing experience in a mental health, behavioral health, or substance use treatment setting preferred.
  • Knowledge of psychiatric medications, recovery-oriented care, and integrated behavioral healthcare.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.
  • Strong assessment, communication, and organizational skills.
  • Proficiency with electronic health records (EHR) and Microsoft Office applications.
  • Valid New York State Driver License with an acceptable driving record.
