About the Role
Matrix providers conduct comprehensive health assessments in members' homes or skilled nursing facilities. These visits provide deep insight into a member's health, medical history, and social risks, helping to improve care quality and close health gaps.
Responsibilities
- Conduct adult and geriatric assessments including medical history, diagnosis, and treatment
- Provide health education, physician referrals, and case management referrals
- Perform follow-up and maintain clear documentation according to protocols
- Administer and order point of care and lab-based testing
- Coordinate with physicians, case managers, social workers, and caregivers
- Collaborate with Primary Care Physicians on patient education and follow-up
Requirements
- Master’s Degree or commensurate experience
- Current NP licensure in state of practice with prescription authority
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology, Family, or Emergency specialty
- Current BLS, ACLS, or CPR certification
- 1 year of NP experience
- Ability to travel 80-90% of the time, including overnight and weekends
- Valid state driver's license
- Automobile insurance with specific coverage limits for bodily injury and property damage
- Strong computer skills and familiarity with medical record software
- Excellent verbal and written communication skills

- $1,800 – $11,500 per month based on visit volume
- $90 – $115 per assessment
