About the Role

Matrix providers conduct comprehensive health assessments in members' homes or skilled nursing facilities. These visits provide deep insight into a member's health, medical history, and social risks, helping to improve care quality and close health gaps.

Responsibilities

  • Conduct adult and geriatric assessments including medical history, diagnosis, and treatment
  • Provide health education, physician referrals, and case management referrals
  • Perform follow-up and maintain clear documentation according to protocols
  • Administer and order point of care and lab-based testing
  • Coordinate with physicians, case managers, social workers, and caregivers
  • Collaborate with Primary Care Physicians on patient education and follow-up

Requirements

  • Master’s Degree or commensurate experience
  • Current NP licensure in state of practice with prescription authority
  • AANP, AACN, or ANCC board certification in Adult, Adult Gerontology, Family, or Emergency specialty
  • Current BLS, ACLS, or CPR certification
  • 1 year of NP experience
  • Ability to travel 80-90% of the time, including overnight and weekends
  • Valid state driver's license
  • Automobile insurance with specific coverage limits for bodily injury and property damage
  • Strong computer skills and familiarity with medical record software
  • Excellent verbal and written communication skills

  • $1,800 – $11,500 per month based on visit volume
  • $90 – $115 per assessment
