About the Role

Experienced Case Manager to address the preparatory needs of participants in the early stages of recovery, develop daily living skills, and coping mechanisms through group and individual work in an OASAS funded program.

Responsibilities

  • Provide case management services to a caseload
  • Manage and develop systems in the facility
  • Complete all required charting paperwork for the funding source
  • Conduct groups periodically or as directed by management
  • Provide Housing Assistance to clients
  • Complete Housing Assessments and Referrals
  • Complete HRA 2010-E’s
  • Attend management trainings, team meetings, and staff meetings
  • Provide assistance to clients within the facility when needed
  • Performs other duties as required by the Program Director and Clinical Supervisor
  • Other duties as assigned

Requirements

  • Full CASAC or Bachelor degree OR CASAC-T with Bachelor degree
  • 2 years of case management work experience in a social service agency, preferably serving mental health or substance using populations

Skills

  • Knowledge of substance use, mental illness, treatment options, and the needs of an individual integrating back into the community
  • Excellent written, verbal and computational skills to present and document records in accordance with program standards
