About the Role

The Hotspotting Opioid Program is an evidence-based program led by certified recovery peer advocates and clinical professionals. It provides person-centered services to those at risk of opioid overdose, aiming to reduce harm, promote recovery, and address social determinants of health.

Responsibilities

  • Provide client outreach and interventions.
  • Offer crisis consultations.
  • Maintain client case records.
  • Facilitate linkages to care and referral follow-up.
  • Document interventions according to funder and agency requirements.
  • Submit statistical data and reports.
  • Attend weekly supervision and participate in case conferences.
  • Conduct staff trainings on substance abuse topics.
  • Perform outreach to engage potential clients into services.
  • Coordinate the implementation of the multidisciplinary treatment plan with the treatment team.
  • Document all client activities, progress, and clinical interventions in the case record.
  • Coordinate treatment services with internal programs and external organizations.
  • Address and remove barriers to client access to healthcare, including mental health and substance use services.
  • Maintain ongoing contacts to support client compliance with treatment and services, mitigate barriers, and assist with employability.
  • Use required systems to track outreach and engagement activity.
  • Conduct Health Related Social Needs Assessments (HRSN) and make referrals.
  • Assist clients in coordinating manageable treatment.
  • Help collect documentation for SSI and housing applications and provide advocacy.
  • Coordinate logistics for care and treatment needs, including appointment reminders and follow-up.
  • Assess and plan treatment adherence, addressing barriers.
  • Connect clients to Harm Reduction services, including naloxone training and kits, and fentanyl test strips.
  • Reassess client needs and eligibility for the program.
  • Develop and discuss aftercare plans with clients, referral resources, and other entities.
  • Perform crisis intervention and problem resolution for clients.
  • Coordinate efforts with treatment staff and document interventions.
  • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values.
  • Perform other duties as assigned.

Requirements

  • At least 1-year of experience providing case management services.
  • Experience working with individuals managing substance use disorders.
  • Bachelor’s degree in a related field, CASAC, or CASAC-T preferred.
  • CRPA Preferred.
  • Strong knowledge of harm reduction, trauma-informed care, motivational interviewing, and other person-centered intervention models.
  • Good organizational skills and an orientation toward detail and accuracy.
  • Bilingual (English/Spanish) preferred.
  • Computer literate in Microsoft Office applications, email, and ability to navigate the Internet.
  • Effective communication skills, including ability to follow instructions, listen, communicate effectively in writing and verbally; able to organize thoughts logically and explain self clearly.

  • $41,600 - $46,600 annually
