About the Role

Responsibilities

• Recommend employment decisions for staff, including hiring, firing, advancement, promotion, or status changes.
• Ensure the overall quality of patient care and clinical performance of nurses and aides.
• Organize clinical operations for the care center.
• Review service requests and determine patient eligibility for home care services, coordinating referrals and admissions with intake.
• Oversee scheduling.
• Oversee payor verification and precertification requirements.
• Review staff documentation to ensure timely and complete resolution of issues.
• Assist clinicians in establishing patient goals, setting priorities, and developing Plans of Care (POC).
• Evaluate patient needs and revise the plan of care in collaboration with clinicians.
• Provide direct patient care infrequently in emergency situations.
• Participate in the on-call process.
• Assist with staff training to ensure quality care, compliance, and fiscal responsibility.
• Participate in the investigation and resolution of patient, family, and physician concerns.

Requirements

• Associate or bachelor's degree in nursing.
• Current RN license specific to the assigned state(s).
• One year of clinical RN experience in home health or hospice.
• One year of RN management experience.
• Current CPR certification.

• Monday to Friday, 8:00 A.M. to 5:00 P.M.

Salary/Compensations

• $130,000.00 to $135,000.00
• $20,000 sign-on bonus
