About the Role
The Transitional Housing Worker (THW) plays a vital role in supporting individuals living with mental health challenges who may also be experiencing homelessness, housing instability, or involvement with the justice system. Working within a supportive housing environment, the THW provides direct, client-centered support that promotes recovery, stability, independence, and meaningful community engagement. This includes assisting clients with daily living skills, wellness activities, and goal-setting, while responding to situations that arise during scheduled shifts, including evenings and overnights. THWs are essential contributors in helping clients transition toward more independent living with confidence and dignity.
Responsibilities
- Supporting clients with daily living skills such as meal preparation, personal care, cleaning, and medication reminders
- Support clients through individualized case management, life skills coaching, and service planning to promote successful transitions to independent living
- Foster a sense of community among residents and promote social inclusion and empowerment
- Building positive, trusting relationships to better understand and address clients’ needs while maintaining professional boundaries
- Collaborate with external agencies and internal teams to ensure coordinated care
- Conducting regular unit inspections to ensure health and safety conditions are being kept
- Responding to crisis calls and supporting crisis prevention by ensuring appropriate interventions and follow-ups are provided in a trauma-informed approach
- Maintaining up-to-date and accurate case documentation within the program database, ensuring that client information, progress notes, and service records are consistently recorded
- Collaborating with colleagues and community partners to coordinate care, share information appropriately, and identify opportunities for service improvement
Requirements
- Experience providing community based direct service to adults with mental illness
- The ability to make informed decisions in dynamic situations, and apply sound judgment when supporting clients with complex needs
- Knowledge of the criminal justice system, concurrent disorders and issues affecting individuals experiencing homelessness
- Familiarity with homeless outreach initiatives and the shelter system
- Ability to foster and maintain relationships with the corporate sector and community agencies
- Individuals seeking meaningful, client-focused work and who are comfortable taking initiative, adapting to changing situations, and supporting the smooth and effective operation of a housing program
- Candidates will be required to adhere to our COVID-19 policy and provide proof of vaccination
Skills
- Degree/Diploma in Social Work, Social Services or related field in human services
- Good written and verbal communication skills
- Excellent interpersonal and conflict resolution skills
- Experience working with diverse communities
