About the Role
FOX Rehabilitation is seeking passionate, licensed Physical Therapists in Goshen, NY, to provide exceptional one-on-one care to older adults in their homes or senior living communities. This role emphasizes autonomy, flexibility, and meaningful patient outcomes through the Geriatric House Calls™ model, with opportunities to incorporate telehealth.
Responsibilities
- Provide one-on-one physical therapy to older adults in private residences or senior living communities.
- Perform evaluations and develop individualized, evidence-based plans of care.
- Deliver therapy sessions focused on patient wellness, longevity, mobility, and functional independence.
- Document patient care effectively within the EMR system.
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Provide telehealth/virtual care to enhance access and flexibility.
Requirements
- Valid Physical Therapy license in the state of practice, or eligibility to obtain.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet physical demands of in-home and community-based care, including lifting, patient transfers, standing/walking for extended periods, climbing stairs, and driving.
- Must be able to wear PPE when required.

Salary/Compensations
- Full-Time: $75,000 - $115,000 annually
- Part-Time and FOX Flex (PRN) will be pro-rated.
