About the Role

Bristol Hospice is seeking dedicated RN Case Managers passionate about providing compassionate, whole-person care and advocating for patients and their families during their hospice journey. The role focuses on delivering meaningful end-of-life care with respect and dignity.

Responsibilities

  • Develop and implement individualized care plans based on current and prior medical history
  • Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
  • Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
  • Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
  • Administer physician-ordered medications and treatments with skill and empathy
  • Educate patients and their families on care techniques and symptom management
  • Track and communicate changes in patient condition, ensuring timely and effective responses
  • Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
  • Collaborate with interdisciplinary teams to ensure comprehensive care delivery
  • Contribute to other supportive duties as part of a compassionate, mission-driven team
  • Perform additional duties as assigned

Requirements

  • A valid Registered Nurse license in good standing in the state you will work in
  • A valid driver’s license and reliable transportation
  • CPR certification (current)
  • Strong observational and communication skills—both written and verbal
  • A flexible, team-oriented attitude and willingness to support additional duties as needed
